Используйте сайт QForm.Link
Создайте форму и дайте ей название. Оно будет отображаться в качестве заголовка формы
Добавьте и настройте типы полей:
Разместите «Текстовый блок».
Поле поддерживает теги HTML, используя которые можно добавлять элементы и задавать стили внутри тегов. В нашем случае это заголовок и полезная информация для клиента
Добавляйте поля, исходя из целей вашей формы, которые помогут собрать более точную информацию о желаниях клиента. У нас в примере это размещенные в одной строке поля «Дата» и «Время», которые вносят детали в заявку клиента 
Добавьте поля «Имя» и «E-mail» для указания данных, кому и куда будет направлен ответ от менеджера

Выборочно или все типы сделайте обязательными для заполнения
Кликните по кнопке ссылки, чтобы открыть готовую форму
В открывшемся окне скопируйте адрес и делитесь ссылкой на форму

После этого все заявки будут приходить во внутреннюю CRM и храниться в вашем личном кабинете
